[The completed application form for the Academic Excellence Award should be submitted to the COTA office no later than September 1st annually.] PDF/Application Form-Academic Excellence Award.pdf Students must complete an application form (available from the school principal or the COTA office) and forward a copy of their Ministry of Education Grade 12 Provincial Examination Scholarship results form and acceptance to a post-secondary institution to the C.O.T.A. office no later than September 1 following the applicant's Gr. 12 graduation. Recipients of previous C.O.T.A. scholarships are not eligible for the Academic Excellence Award. A. S. MATHESON EDUCATION SCHOLARSHIP - $1,000: [The completed application form for the A.S. Matheson Education scholarship should be submitted to the COTA office no later than September 1st annually.] PDF/Application Form-AS Matheson Education.pdf Students graduating from Grade 12 are made aware of this scholarship should they be accepted to an education program. The scholarship is advertised in the Universities of B.C. Potential candidates should contact the Financial Awards office of their post-secondary institution for further details. This scholarship will be awarded in the Fall of each year to the son or daughter of a current or retired C.O.T.A. member presently enrolled at a post-secondary institute and who is proceeding into the first year of a teacher education program. Candidates must submit a completed application form to C.O.T.A. by September 1; a letter of acceptance to the Faculty of Education and Teacher Education Program, copy of university student transcript; a personal letter with details of past achievements and future goals; and 2 personal letters of recommendation must also be attached.
